Who may qualify

You may qualify if you have NY Medicaid, live in Brooklyn, the Bronx, Harlem or Queens, and one of these fits you:

  • You are pregnant or just had a baby
  • You live with a long-term health condition, like diabetes or heart disease
  • It is hard to get enough healthy food at home
  • Other needs — ask us, we will check
